Finance & Insurance


To answer the most common question from patients:  Yes our practice accepts insurance.  We use both PPO Plans and also will work with most HMO and Indemnity plans. 


Your Dental Insurance I.D. card has your policy's toll-free contact # on the reverse side. Be sure to contact your Provider for coverage details at the time of service.  Double checking that your policy is in effect could save you a lot of time when in the office.


When you come in, if you want us to use your Dental Insurance, please have your card (or a copy of the front and back of the card), plus the following information:

     * Who is the insured?  (this will almost always be either the patient, or spouse).

     * Social security of both the patient & the spouse (if your insurance is through your spouse's          employer.

     * Date-of-Birth of both the patient & the spouse (if your insurance is through your spouse's            employer.

     * Group name/number if different from Social Security #.

     * If you would like to expedite the process of validating your Insurance, you may elect to take        a picture with your cell phone of the front and back of your Insurance Card, and send this to        us either by e-mail, text message, or fax (our fax# is: (904) 296-0147).

PPO Insurance:

PPO Insurance is simply a Dental Insurance Plan where your Insurance Company pays your dental provider a fee for dental procedures that is based on both a percentage factor of their own "internal" Network Fee List, not the Dentist's fees -- ALSO, all PPO Plans have an "Annual Maximum" which caps the amount of reimburesement from any one policy plan on an annual basis.

These factors are all based on the contracted employer (this means the company who has provided a policy to their empolyees), and these factors vary widely between plans --- it is not the Insurance Company that sets a "one-size fits all", but rather the policy plan that your (or your spouse's) Employer selected.


This Network Fee List is carefully protected by the PPO Insurance Companies, and 


At the bottom of this page is an list of PPO insurance plans that we work with.  If your plan isn’t listed, please call us to check your coverage.   We will get on any PPO Insurance list -- just tell us what you have and we will handle the rest.

HMO (DHMO) Insurance:

A commonly misunderstood factor with HMO Insurance is that you have to be treated by your "Assigned Dentist".   For most general dentistry treatment services this applies, but does not for Dentures and Surgical related procedures in our practice.

You do not need to go to your "assigned" dentist to receive your HMO discounted co-payment fees with our practice.

Even if we are not a participant with your particular plan, we will honor your plan's schedule of co-payment fees.

All you need to do is to bring your Insurance Card with you, and also bring a copy of your plan's Co-Payment fees.

These are exactly the same fees that you will be required to pay with your assigned dentist.

For Denture & Surgical treatment, it is just not necessary for you to have to go to a Dentist assigned to you by your Insurance Company
